Nurse Practitioner/ Physician Assistant, Northside Elementary School-Based Health Center
(1 FTE), Montrose County School District Re-1J, Montrose, CO (Posted February 2, 2008)

Role: The nurse practitioner/physician assistant will work with pediatric clients enrolled in the grant funded SBHC and their families.  They will be responsible for providing well and sick care, with appropriate referrals and consultation to medical collaborating/supervising physician.  They will lead the SBHC team in achieving program goals.  

Duties and responsibilities:

  • Provides sick and well visits, and patient teaching for children enrolled in the SBHC.

  • Collaborates with local community providers, SBHC staff, and school personnel for coordination of care.

  • Develops and implements health education and health promotion activities.

  • Directs Continuous Quality Improvement (CQI) activities.

  • Acts as team leader for center staff and providers.

  • Facilitates communication between school district and clinic staff.

  • Maintains current continuing educational hours, certification and licensure.

  • Serves as clinical preceptor as needed for nursing students.

Qualifications:

  • Graduation from an accredited NP or PA program.

  • Current licensure in Colorado as an Advanced Practice Nurse with a specialty of Family Medicine or Pediatrics or Licensure as a Physician Assistant

  • Experience in primary health care to children

  • Current DEA certification.

  • Current certification by appropriate credentialing body

  • Bilingual Spanish/English preferred

  • Familiarity with community agencies and resources

Additional administrative duties assigned may include, but not be limited to: 

  • Manage agreements/contracts with outside agencies, 3rd party payers

  • Seek continuing funding sources, community support including grant writing

  • Report to funders, Community Advisory Council, and school district

  • Administer clinic budget

Nurse Practitioner/ Physician Assistant, Tlatelolco School-Based Health Center
(PT), Escuela Tlatelolco, Denver, CO (Posted May 19, 2008):

The Rocky Mountain Youth Clinics is seeking a Nurse Practitioner or Physician Assistant to work 2 to 3 half days on a year-round schedule. Bilingual (English and Spanish) is preferred.

The school of 150 students, emphasizes experiential learning and prides itself on its philosophy of social justice and Aztec/Chicano heritage. About 90% of students are equivalent of free and reduced lunch and come from all parts of the city. Pre-K is part of the Colorado Preschool program, elementary is Montessori-based through grade 8, and the upper school is a multigrade DPS drop-out prevention school.

This year the clinic/school is collaborating with Colorado Organization on Adolescent Pregnancy, Parenting, and Prevention (COAPPP) in implementing a reproductive health education/HIV prevention program, which supports ½ day of extra clinic time for outreach and testing.   The Rocky Mountain Youth Clinics provides dental care; mental health services are provided by volunteer social worker.

RN Nurse Practitioner or Physician Assistant
(1 FTE) Aurora Public School District, Aurora, CO  (Posted May 22, 2008)

This position is available for a nurse practitioner or physician assistant to work in a new year-round Aurora school-based health center providing primary care services to elementary school aged children.  This person acts as the team leader for clinic staff and in consultation with supervising physician. Current CO Physician Assistant or Nurse Practitioner license required.

Medical Assistant
